Remember, all of the forms below are required.

It is YOUR (the student) responsibility to make sure we receive your teacher recommendation forms, counselor course/grade verification form, and parent/guardian acknowledgment form in addition to the student application form before the deadline date.

• Ask your science teacher and math teacher to each submit the recommendation form on your behalf. Be clear that the form must be received before the deadline. Tell them to which Holland program you are applying: SEP, SSRP, or RISE. Give them the URL (go.iu.edu/holland-teacher-form) so that they can access the recommendation form.
  
  Teacher form
• Ask your school counselor to submit the course/grade verification form on your behalf. Be clear that the form must be received before the deadline. Tell your counselor to which Holland program you are applying: SEP, SSRP, or RISE. Give your counselor the URL (go.iu.edu/holland-counselor-form) to access the verification form.
  
  Counselor form
• Ask your parent/guardian to submit the parent/guardian acknowledgment form on your behalf. Be clear that the form must be received before the deadline. Tell your parent/guardian to which Holland program you are applying: SEP, SSRP, or RISE. Give him/her the URL (go.iu.edu/holland-parent-form) to access the acknowledgment form.
